Analyze the MnCHOICES Assessment submitted by the lead agency to ensure a thorough understanding of the client’s needs. 

 Extract critical details from the assessment to shape the CFSS Service Delivery Plan. technology to deliver the best possible care to our clients.

 Engage with clients to collaboratively design the service plan and provide them with education on CFSS options and processes. 

 Upload the completed service plan to the client's MnCHOICES record.  Inform the lead agency upon finalizing the service plan. 

Offer ongoing assistance and support to the individual as needed.
